Please use this form to request use of the Calvary facilities

Does your event require audio-visual (AV) equipment or support?

  • Building Fee: $400 up to 4 hours ($75 each additional hour)
  • Cleaning Fee: $125 
  • A/V Staff Fee IF NEEDED: $175 up to 4 hours ($50 each additional hour)

Calvary Church Materials

** Event host will be responsible for all event supplies (ex. tableware, cutlery, food, etc.)
** Any Calvary Church materials or furniture used must be put back in their proper places.
** Kitchen supplies used must be washed, dried, and put away.

Disclosure

** Calvary Church is not responsible or liable for any injuries or property damage during the event. All Calvary items must be returned in the same condition it was received. Event host will be responsible to replace any damaged items.

Approval

Upon submitting your event, a Calvary Staff will follow up with approval and any further clarifying questions.
